INTRODUCTION

Tony Mapu attended Southeast Polk High School in Altoona, Iowa intermittently from 1977-
1980. He and his family traveled the country as an entertainment group known as “The Sounds
of Polynesia”. On October 25 th , 1980, Tony’s life ended his senior year of high school, in a tragic
car accident. He left a very large impact on his classmates, family and friends throughout the
district, community and beyond. He was an exemplary student-athlete.

The Tony Mapu Memorial Foundation’s Vision is to inspire, empower and equip young people
for excellence in academics, athletics and arts. Scholarships will be awarded to student athletes
through an application review, interview and approval process. The foundation is seeking
candidates who demonstrate efforts and accomplishments proving the will to move out of their
comfort zone to an elevated level of contribution and performance in line with Tony’s life.

Tony Mapu Memorial Foundation Scholarship candidates are expected to be:

Awards are for undergraduates only.

scholarships. You will need to enter a parent email address to complete the consent process***

ELIGIBILITY CRITERIA

1. Upcoming Graduating Southeast Polk high school Senior
2. GPA of at least 2.75 or higher
3. Must attend a higher educational institution
4. Student R

5. Full-time course enrollment certificate from institution

DIRECTIONS ON HOW TO COMPLETE THE APPLICATION

Tips for application preparation:

  1. Read and follow all guidelines.​

  2. Think carefully about how you want to respond to the essay questions before you begiwriting.

  3. Proofread all your work thoroughly. Responses that are unclear or contain numerous mistakes distract the reviewer from focusing on the content and reflect poorly on your ability to produce a high-quality product. Make sure that sentences are complete, clear, and grammatically correct.

  4. Have someone else read your completed application. An objective reader can often identify areas that need more explanation and may find errors that you have overlooked.

  5. You will need to attach the letters of reference and essay questions separately.

SUPPLEMENTAL DOCUMENTS (Semi-finalists Only)

If you are notified that you have been selected as a semifinalist, you will be required to send in the following documents and video.

  1.  Current high school transcript. (If you have attended more than one high school and your current transcript does not reflect all academic years, request a transcript from each school you have attended to document your entire high school career.)

  2. Copy of your Student Aid Report (SAR)

  3. Video interview submission for final scholarship selection criteria provided by TMMF.

AWARD INFORMATION

TMMF is awarding 10 - $2,000 scholarships in 2024. Recipients will receive the scholarship in a one-time installment payment from TMMF paid directly to the scholar’s institution on behalf of scholar, based on the student submitting all necessary items from the institution.

SCHOLARSHIP RECIPIENT REQUIREMENTS

  • The award recipient understands it is their own responsibility to provide
    TMMF an unpaid institutional student account invoice/statement two weeks prior to due date. The account invoice/statement.

    • The statement must contain verifiable student identification, an account invoice/statement date, and a due date to be considered for scholarship award payment.

  • Failure to provide required timely submission of documentation above may result in delay or revocation of award.

  • Award recipients understand and agree that any award payment will be made only to the institution, towards the account balance of the award recipient.

  • Award recipients understand and agree that any portions of TMMF scholarship award monies, for whatever reason, if refunded, will be returned by the institution payable only to the TMMF
